Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan.
Payment
Payment in full is requested at the time of each visit; I accept checks or cash. If needed, I can furnish you with a statement for possible reimbursement from your insurance company.
Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full fee for the missed time. The only exception to this policy will be during periods of inclement weather or hazardous driving conditions (your call). Cancellations are accepted by voicemail only - not through email - as I am more likely to check phone messages more frequently.
